Food Protection Program Applications

·         Plan Review for Fixed Food Establishments

•      Ø Required for newly constructed or extensively remodeled fixed   
•                   food establishments.
•  

·         Health Permit Application

Ø  Required to operate a fixed or mobile food establishment in McHenry County.

·         Supplemental Outdoor Grilling Permit

Ø  Required in addition to the regular health permit to do outdoor grilling at a fixed food establishment.

·         Plan Review for Mobile Food Establishments

      Ø  Required for new or extensively remodeled mobile food  

                     establishment.

·         Temporary Food Event

Ø  Required to operate a Category 1 or 2 Temporary Food Establishment.

Onsite Wastewater Disposal System (Septic System) Applications

·         Septic System Permit  

Ø  Required to construct, repair, modify, replace or abandon an onsite wastewater treatment system (septic system).

·         Health Review

Ø  Required to obtain a written evaluation from the Health Department of the impact of an addition, remodeling project or accessory structure on an existing onsite wastewater treatment system.

·         Site Evaluation

Ø  Written evaluation of the suitability of a site to install a compliant onsite wastewater treatment system to serve a new residence or structure.

·         Septic System Evaluation

Ø  An evaluation of the current operation of an onsite wastewater treatment system typically done for real estate transactions and/or refinancing of mortgages.

·         Septage Management Site Permit Application

•                 Ø Required to store, dispose or use septage at any site in McHenry County except                   an Illinois Environmental Protection Agency permitted sewage treatment plant.
•  

·         Septage Management Site Evaluation Form 

                     Ø An evaluation of the feasibility of a site to meet Ordinance requirements for use

                          as a septage management site.

·          Septage Pumping Contractor Permit Application

                   Ø Required to collect, store, transport, dispose or use septage, portable toilet waste

                             or domestic sewage from a septic tank, holding tank, privy vault or sanitary dump

                             station.

Private Water Supply Applications

·         Water Well Permit  

Ø  Required to construct, repair, modify, replace or abandon a water well in McHenry County.

·         Water Well Evaluation

Ø  An evaluation of the current operation of a water well typically done for real estate transactions and/or refinancing of mortgages.  Includes analysis of water sample for coliform bacteria and nitrates.

·         Illinois Department of Public Health Non-Community Well Application

•      Ø  Required by Illinois Department of Public Health to construct or modify a
•                 non-community public water supply.

·         Illinois Department of Public Health Application to Construct, Modify or Abandon Well

Ø  Required by Illinois Department of Public Health in addition to the county

   permit to construct, modify or abandon a water well.

·         Chain of Custody Form

                  Ø  Must be completed and submitted to the laboratory when a water sample is

                          submitted for analysis.

Open Burning of Landscape Waste

·         Bonfire Permit

                  Ø Required for an outdoor ceremonial fire as defined in Article III of the McHenry

                            County Public Health Ordinance.

·         Application for Habitat Reclamation Permit (IEPA)

                  Ø Required by the Illinois Environmental Protection Agency for controlled burn for

                            habitat reclamation.
