 DOCTOR EXAM:

• Serve infants and children through 18 years of age
• Clinics are staffed by Physicians and Public Health Nurses
• Routine well child physicals, school physicals, and sports physicals
• Hemoglobin and urinalysis performed if needed
• Routine childhood immunizations provided 
• Age appropriate developmental assessment with referrals
• Lead risk assessment and blood test performed if indicated
• Sliding scale fee, Medicaid/All Kid's accepted
• Parents/legal guardian MUST accompany child, bring shot record to visit

 Together We Can (TWC) - Immunization Partnership Clinics:

• All scheduled childhood vaccines are available
• Immunization for infants and children (6 weeks to 18 years of age) through Vaccine for Children (VFC) program (uninsured, underinsured and Medicaid/All Kid's recipients)
• 13 clinics held per month
• 11 total sites located throughout McHenry County
• Hours vary - some evening hours
• By appointment only
• VFC immunizations are $15 each, cash or check, Medicaid/All Kid's card accepted
• No one is denied for inability to pay fee

• Medicaid/All Kid's card if applicable
• Parent/legal guardian MUST accompany child under 18 years of age
