Family Case Management (FCM) Program
What is the purpose of FCM?
Family Case Management promotes optimal health and provides support for pregnant women, infants, and children with high-risk medical conditions by encouraging early prenatal care and making referrals to community and social services.
Who can receive FCM services and what does it cost?
If you are pregnant, have a child under the age of one, are on Medicaid, or part of a low-income family you may be eligible. There is NO COST to clients for services provided.
What services does the FCM program offer? |

Family Case Management can help you:
· Find a doctor for your prenatal care to make sure you have a healthy baby
· Understand proper nutrition for you and your child
· Understand the stages of your child's development
· Get information on how to become a better parent
· Understand the importance of prenatal care
· Learn the signs and symptoms of 0-term labor
· Understand the importance of regularly scheduled well child visits
· Understand the importance of immunizations

Our dedicated Family Case Management Public Health Nurses and Case Managers can see you in the office and in your home. They will provide information related to prenatal needs, infant care and parenting. Home visiting services provide the opportunity to learn more about household safety tips to protect your infant or child.
