Home Health Care

 

The Home Health Care Program provides professional health care services to residents with physical and/or behavioral health care needs in their home.  Skilled nursing; health aide; physical, occupational, speech therapies; and social work services may be provided and are directed by the individual resident physician and/or psychiatrist.  Services are covered by the Mental Health Board, Medicare, Medicaid, Office of Rehabilitation Services or adjusted fee.

Skilled Nursing Services:

  • All home care cases start with a skilled nursing assessment.
  • Registered nurses maintain the vital link between client and physician.
  • Registered nurses co-ordinate the total home care team.
  • Nursing services are provided on an intermittent visit, rather than an hourly basis.
  • Services provided may include: teaching, monitoring and clinical nursing practices.

 

Home Health Aides:

  • Home Health Aides are certified by the State of Illinois and work under the direct supervision of a registered nurse.
  • They can assist with:

           * Bathing and shampooing

           * Dressing and grooming

           * Ambulation/therapy exercises

           * Reporting changes in condition/needs to the registered nurse

 

Therapy Services:

  • Comprehensive therapy services including physical therapy, occupational therapy and speech therapy are available.
  • Licenses professionals work under the direction of a physician.
  • Programs are designed to:

          * Restore function
          * Diminish pain
          * Minimize disability
          * Evaluate and modify living areas

 

Clinical Social Worker:

  • Services from a licensed clinical social worker are also available.
  • Assistance and guidance can be obtained for clients regarding their mental health, financial, housing and other socials service needs.

 

Sustaining Care Program:

The Sustaining Care Program provides in-home treatment services to sustain and improve the level of functioning of impaired individuals with primary diagnosis of:
          * Mental illness

          * Developmental disability

          * Substance abuse

          * Traumatic brain injury

 

The primary goal is to prevent deterioration of the behavioral health condition. Clients receive intermittent teaching, monitoring and supportive skilled nursing visits. We encourage participation in other Mental Health Programs and provide linkage whenever necessary.

 

This program is funded by the McHenry County Mental Health Board.
